Application Guidelines
 
Submission Deadline: 5:00 p.m. (Eastern Time) April 1, 2012
 
Your application information will be used by the Maureen and Mike Mansfield Foundation exclusively in the selection process and will not be distributed.
1. Review the guidelines and eligibility requirements thoroughly before applying. These sections cover many administrative, professional, financial, and personal issues that may greatly affect your decision to apply for—or ability to accept the obligation of—a Fellowship.
2. Read all forms before completing them. Do not skip anything.
3. Answer questions as completely as possible in the space provided. Add a supplementary sheet only if requested or if you feel it is essential to answer a question fully. Clearly identify the form that each sheet is supplementing by typing that form’s name at the top of the separate sheet.
4. TYPE your responses on any separate sheets on standard white 8 ½” x 11” paper.
5. Your typing must be sufficiently dark to photocopy clearly.
6. Use only one side of any separate sheets you submit.
7. Collate the material in the order shown in the checklist below, three-hole punch, and bind with a clip in the upper left hand corner. Do not staple or submit materials in any kind of binder or cover.
8. Submit one original and  six complete copies of the application and supplementary materials to the Washington, D.C. office of the Maureen and Mike Mansfield Foundation.
9. No applications or reference reports may be sent via email, disk or by fax.


NOTE: All applications become part of the Mansfield Foundation’s files and cannot by returned. Keep a copy!

Reference Report Please give a copy of the three-page Reference Report form—with Part 1 typed, signed, and dated by the applicant—to each person from whom you request a recommendation.
